Using Narrow Band Imaging in Colonoscopy

Using Narrow Band Imaging in Colonoscopy
Narrow Band Imaging (NBI) has been available on Olympus colonoscopes for 15 years. However, many clinicians never or almost never use this feature. The use of NBI can improve detection of adenomas and serrated lesions, it allows accurate differentiation of serrated lesions from conventional adenomas, and it facilitates identification of deep submucosally invasive cancer that contraindicates endoscopic resection. NBI also allows accurate assessment of resection scars for recurrence vs clip artifact and allows distal colon hyperplastic polyps to be left in place without resection. NBI can serve as the basis for the widely discussed resect and discard policy, which is established as safe and cost-effective but is not used in practice because of lack of financial incentives and perceived medical legal risk. Listen to Doug Rex review the use of NBI and what considerations clinicians should think about in its use in practice.

FACULTY:

Douglas K. Rex, MD, MASGE
Distinguished Professor Of Medicine
Indiana University School of Medicine
Indianapolis, IN

Dr. Douglas Rex’s clinical interests are in endoscopy, particularly colonoscopy. He has the largest experience in endoscopic resection of large colon polyps in the United States, and the world’s largest experience in completing colonoscopy in patients with previous incomplete colonoscopy.

OBJECTIVES:

  1. Understand clinical uses of narrow band imaging (NBI) in colonoscopy procedures to enhance the detail of certain aspects of the surface of the mucosa.
  2. Compare and Contrast the use of narrow band imaging (NBI).
